To submit a T-Shirt order online, please open the order form below and fill out all of the required fields in the form and click the "Submit" button.

When you click "Submit" either select "Desktop Email Application" and send the email with your attached order; or select "Internet Email" 

If you select "Internet Email" you will be prompted to save your order form on your computer. Please save your completed order form and then open your online email account and attach the saved order to an email to: accounting@u-fli.com 

If you want to fax your order, please fax to (818)990-2621. 

If you have any problems placing your order, just email accounting@u-fli.com . We will make it work.

Please submit all orders no later than August 7, 2012.
